Post-acute programs offer patients a short-term rehabilitation and nursing option following the fragile phase of an illness or injury. In this level of care, patients receive additional medical monitoring and rehabilitation before returning to their prior living setting.
Southport Health & Rehabilitation Center is part of Liberty Healthcare and Rehabilitation Services, a network of post-acute skilled rehabilitation facilities. Since 1870, the McNeill Family has developed and operated health care services in North Carolina. This post-acute care delivery system extends beyond our skilled nursing facilities to include pharmacy, assisted living, home health, hospice, and medical equipment.
