Creswell Health and Rehabilitation Center is a cozy building perched on a little hill, just high enough to look over the city. Our center is easily accessible and located within one mile of Interstate 5.

Our staff is genuine and we take time to honor the unique and individual qualities of every resident. At Creswell Health and Rehabilitation Center, we prize quality, one-on-one time with our residents and often share stories and light-hearted fun during group activities or around the dinner table.

Our newly remodeled nursing and rehabilitation center offers a choice of private and semi-private rooms. Our rooms are comfortable and our 24-hour care team strives to make our center a homey environment for all residents.

Creswell Health and Rehabilitation Center is a contracted provider for Medicaid, Medicare and several managed care health plans.
